Since many of you have asked me to read letters you have written, I have found out that addressing the District Attorney must be brought up. Proper Letter format for addressing the Office of the District Attorney. Or cover letter if you are sending him/her an affidavit. Your Name Your Address Town/State/Zip Tel: Fax: Date: Honorable John Doe District Attorney County Of __________ Street Address Town/State/Zip Tel: Fax: Honerable John Doe, I am repectfully submitting a sworn affidavit to your office, which is attached to this cover letter. What I am praying is that your office investigate the matters atttested to. OR Please keep this on file as it attests the accurate history of facts in a case in _________Court in your County. Sincerely, Your Name Typed (Sign OVER your typed name) CC: File (You keep a copy) Attorney (if you have one) (After you satisfy your file and attorney, list the rest of the CC's by order of rank importance. IE: don't list the Judge AFTER a police officer. A Judge would be rather high on this cc list) On the address lines, I put the phone and fax numbers for all parties. I add them on the cc's to, for most. This is so someone can call me if they need to. Letters that have proper contact information get attention faster. Putting the DA's tel/fax #'s is for your reference. If you need to call or fax, you have this right in front of you. No fumbling through the phone book each time you pull a letter out of the file to follow up on) The "Honorable" part is showing respect to the Office. The elected DA. Not the ADA's.
